A governance vacuum exists whenever an AI system is deployed without execution-layer governance. The system runs, takes actions, and transforms signals — but nothing checks those actions against codified rules before they execute. The gap between deployment and enforcement is the vacuum. Inside it, transformations degrade the semantic commitment of the signals they touch: C(T(S)) < C(S). Over time, this degradation compounds, producing unreliable, unaccountable, and unauditable operations.
